About The Position

This is a temporary travel assignment for a Physical Therapist (General) in Lewistown, Missouri, within a Skilled Nursing Facility. The role involves evaluating residents, developing individualized treatment plans, and delivering skilled physical therapy services in a team-based, long-term care environment. The position requires collaboration with an interdisciplinary team, adherence to physician orders and facility policies, and compliant documentation. The assignment is for 13 weeks, starting on 09/21/2026, with a shift of 5x8 Days.

Requirements

  • Current Physical Therapist license in the state of Missouri
  • Graduation from an accredited Physical Therapy program
  • Experience in SNF or geriatric rehabilitation preferred
  • Strong clinical, communication, and organizational skills
  • Ability to work independently and as part of an interdisciplinary team
  • Eligibility to work in a Skilled Nursing Facility/long-term care setting

Responsibilities

  • Conduct initial and ongoing assessments of residents’ mobility, balance, strength, gait, and functional limitations.
  • Develop and implement person-centered treatment plans to improve function, reduce pain, and prevent disability.
  • Deliver direct therapy interventions, including therapeutic exercises, gait training, balance retraining, and use of modalities as ordered.
  • Monitor progress and adjust care plans to achieve prescribed therapy goals.
  • Participate in interdisciplinary team meetings and contribute to discharge planning as appropriate.
  • Educate residents, families, and caregivers on home exercise programs, safety strategies, and activity modification to promote independence.
  • Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides in accordance with licensure requirements.
  • Maintain accurate, timely documentation of treatment sessions, progress notes, and billing information; ensure compliance with state, federal, and facility standards.
  • Support quality improvement initiatives and contribute to ongoing rehabilitation program development.
