About The Position

This is a travel assignment for a Physical Therapist in Tillamook, OR, starting on 10/05/2026 for 24 weeks. The role guarantees 40 hours per week on a 4x10-hour shift schedule. The position is located in a rural, critical access hospital setting where the Physical Therapist will collaborate with an interdisciplinary team to restore and optimize patient function.

Requirements

Doctor of Physical Therapy (DPT) required; only if licensure obtained on or after January 2016, or foreign equivalent
Physical Therapy licensure in the state of practice required
CPR/BLS certification from an approved vendor required
At least two years’ recent field experience
Current nursing license per state of assignment
Ability to work in a travel assignment in a rural setting

Nice To Haves

Two years of Physical Therapy experience preferred

Responsibilities

Perform PT evaluations and establish individualized treatment plans to address physical function and mobility goals
Plan and execute therapy interventions using evidence-based techniques; adjust as needed based on patient progress
Co-lead and participate in interdisciplinary patient groups and care planning
Develop and communicate discharge plans; ensure continuity of care with appropriate stakeholders
Document assessments, plans, progress notes, and outcomes in a timely, accurate manner
Supervise assigned assistants and collaborate with other care team members to optimize patient outcomes
