About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to provide quality home healthcare to patients of all ages in Azusa and surrounding areas. This is an independent contractor opportunity with flexible scheduling and coverage areas, no minimum patient requirement, and per-visit compensation based on the number of patients treated. The clinician determines the plan of care and observes, records, and reports patients’ responses to treatment and changes in condition.

FeldCare Connects provides administrative support with patient assignment, communication, scheduling, care coordination, and quality assurance. Clinicians use the Cliniconnects app to accept or decline referrals and an electronic medical record system for documentation. The organization connects clinicians with patients through home health agencies and hospices.

Requirements

• Current, active, unrestricted Physical Therapy license issued by a state board of Physical Therapy; conditional, provisional, restricted licenses, and limited permits are not accepted.
• Completion of an accredited Physical Therapist program.
• Ability and enthusiasm to work with patients of all ages.
• Reliable personal transportation.
• Smartphone.

• Bilingual ability is a plus.
