About The Position

BAYADA Home Health Care is hiring a new graduate Physical Therapist to provide home health visits to adult and geriatric clients in Fayetteville, North Carolina. The role offers structured orientation, dedicated clinical support, and mentorship from experienced physical therapists to support the transition from education to independent practice. Care is provided one patient at a time in patients’ homes, helping them maintain independence and safety.

Requirements

Graduate of an accredited Doctor of Physical Therapy (DPT) program.
Licensed or license-eligible Physical Therapist in the state of practice.
Excellent communication and interpersonal skills.
Valid driver’s license and reliable transportation.

Responsibilities

Evaluate and assess patients’ functional status and mobility.
Develop individualized treatment plans to maximize independence and safety.
Provide skilled therapeutic interventions in patients’ homes.
Educate patients, families, and caregivers on exercises, mobility, fall prevention, and home safety.
Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
Complete timely electronic documentation while maintaining high standards of patient care.
