Physical Therapist - North Plainfield, NJ
About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in North Plainfield, Scotch Plains, Fanwood, and surrounding New Jersey communities. Care is delivered in private residences and senior living settings, including independent living, assisted living, and memory care, through FOX’s Geriatric House Calls™ model. Clinicians help patients improve mobility, independence, wellness, and quality of life. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and work in a community-based, interdisciplinary practice. Clinicians may incorporate telehealth and other virtual care when appropriate. FOX offers full-time, part-time, and PRN/Fox Flex pathways; the posting also notes H1B sponsorship for qualified clinicians and eligibility for new graduates to participate in its Emerging Professionals Mentor Program. Compensation is based on experience and employment type. The stated New Jersey standard base pay range for full-time roles is $70,000–$102,000 annually, with opportunities to earn more; part-time and FOX Flex compensation is prorated. Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
