Physical Therapist - North Plainfield, NJ
About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in North Plainfield, Scotch Plains, Fanwood, and surrounding New Jersey communities. Care is delivered in patients’ homes and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. Clinicians help patients improve mobility, independence, wellness, longevity, and quality of life. Telehealth and other virtual care may also be incorporated when appropriate. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and work in a clinician-driven, interdisciplinary environment. FOX provides full-time, part-time, and PRN/Fox Flex employment pathways; compensation and benefits depend on experience, role, and eligibility. New graduates may be eligible for the Emerging Professionals Mentor Program, and H1B sponsorship is available for qualified clinicians.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly; performing CPR; providing moderate to maximum assistance with transfers and bed mobility; and standing, walking, and climbing stairs.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy to support patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Meet the physical demands of in-home and community-based care, including assisting with transfers and bed mobility, repositioning patients, and conducting therapeutic activities.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
- Wear PPE when required.
