Physical Therapist - Bordentown, NJ
About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Bordentown and surrounding communities, including Columbus, Beverly, Florence, Chesterfield, Burlington, and Westampton, New Jersey. Care is delivered in private residences and senior living communities using FOX’s Geriatric House Calls™ model. Clinicians help patients improve mobility, independence, wellness, and quality of life. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Clinicians may also incorporate telehealth and other virtual care into their schedules. FOX offers full-time, part-time, and PRN/FOX Flex employment pathways. New graduates may be eligible for the Emerging Professionals Mentor Program, and H1B sponsorship is available for qualified clinicians. Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, CPR, patient transfers and bed mobility, repositioning patients, extended standing and walking, stair climbing, and therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Ability to wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document effectively in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Travel to patient homes and senior living communities as part of the Geriatric House Calls™ model.
