Physical Therapist - Delran, NJ
About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in private residences and senior living communities in Delran and surrounding New Jersey communities. Clinicians evaluate patients, develop individualized plans of care, and provide evidence-based treatment to support mobility, wellness, longevity, and functional independence. Care may also include telehealth or other virtual services when appropriate. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Physical Therapists work within an interdisciplinary team and educate patients, caregivers, and senior living staff to support safety, function, and aging in place. The position is part of FOX’s Geriatric House Calls™ model, which includes travel to patient homes and senior living communities. FOX lists full-time, part-time, and PRN/Fox Flex employment pathways. The stated New Jersey standard base pay range for a full-time role is $70,000–$102,000 annually, with the opportunity to earn more; part-time and PRN compensation is prorated. Benefits are available for eligible roles.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including occasional lifting of 25–50 lbs, frequent lifting of 10–25 lbs, and constant lifting up to 10 lbs.
- Ability to perform CPR and provide moderate to maximum assistance with patient transfers and bed mobility.
- Ability to reposition patients, stand and walk for extended periods, climb stairs, and perform therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy to support patient wellness, longevity, mobility, and functional independence.
- Document effectively in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Travel to patient homes and senior living communities as part of the Geriatric House Calls™ model.
