About The Position

Home Sweet Home is seeking a licensed Physical Therapist to provide skilled home health care in Glendale, Peoria, Surprise, and surrounding areas of Maricopa County. The role involves one-on-one care in patients’ homes, supporting recovery of strength, mobility, confidence, and independence. Compensation is per visit, with an advertised earning potential of up to $103,500 annually based on productivity and a listed per-visit rate of $85–$125 USD. A $5,000 sign-on bonus is offered for full-time work. Per-diem, part-time, and full-time schedules are available.

Requirements

• Active Arizona Physical Therapist license.
• CPR certification from the American Heart Association (AHA).
• At least 1 year of clinical experience; home health experience is preferred.
• Strong clinical judgment and time-management skills.
• Reliable transportation, a valid driver’s license, and insurance.
• Strong communication skills and a patient-first approach.

Responsibilities

• Conduct in-home evaluations, start-of-care visits, and follow-up visits.
• Develop and implement individualized treatment plans.
• Improve patients’ strength, balance, mobility, and functional independence.
• Educate patients and caregivers on safety and equipment use.
• Document visits using the WellSky Kinnser EMR.
• Collaborate with physicians, RNs, and the care team.
