About The Position
GrandCare Health Services is hiring a Physical Therapist to provide in-home physical therapy services in the Coachella Valley. The therapist will follow physician plans of care and help patients regain strength, mobility, confidence, and independence after surgery and for other medical conditions. The role offers autonomy, one-on-one patient care, schedule flexibility, and support from a collaborative team. Work is available part time or full time, with pay based on completed visits and visit type and experience.
Requirements
- Current California Physical Therapy License.
- At least 2 years of physical therapy experience.
- CPR certification.
- Valid driver's license and insured vehicle.
Responsibilities
- Perform home health physical therapy evaluations and develop care plans.
- Provide therapeutic treatment and instruct patients and caregivers.
- Monitor patient progress and report changes to physicians.
- Participate in discharge planning and interdisciplinary case conferences.
- Supervise Physical Therapy Assistants in accordance with state regulations.
