About The Position

Home Sweet Home is hiring a Physical Therapist to provide one-on-one home health care to patients in Glendale, Peoria, Surprise, and surrounding areas in Maricopa County. The role focuses on helping patients regain strength, mobility, independence, and confidence, and supporting their safety and quality of life at home.

The position offers a flexible, independent schedule and per-visit compensation of $85–$125, with annual earnings up to $103,500 based on productivity. A $5,000 sign-on bonus is offered for full-time employees. Scheduling options include per diem, part-time, and full-time work.

Requirements

• Active Arizona Physical Therapist license.
• CPR certification from the American Heart Association (AHA).
• At least 1 year of clinical experience.
• Strong clinical judgment and time-management skills.
• Reliable transportation, a valid driver’s license, and insurance.
• Strong communication skills and a patient-first approach.

Nice To Haves

• Home health experience is preferred.

Responsibilities

• Conduct in-home evaluations, start-of-care visits, and follow-up visits.
• Develop and implement individualized treatment plans.
• Help patients improve strength, balance, mobility, and functional independence.
• Educate patients and caregivers about safety and equipment use.
• Document visits using the WellSky Kinnser EMR.
• Collaborate with physicians, registered nurses, and the care team.
