About The Position

Above and Beyond Home Health Care and Hospice Agency, a Trinity Health company, is hiring a Physical Therapist to provide professional, evidence-based care in patients’ homes in the Monticello, Iowa area. Candidates may live near Monticello, Cedar Rapids, and/or Dubuque. The role involves following physician orders, state regulations, and agency policies while helping clients regain independence and improve their quality of life through personalized rehabilitation plans.

The position includes a $25,000 sign-on bonus. The organization is part of Trinity Health At Home, a national, faith-based, not-for-profit organization serving communities across 11 states. It is Medicare-certified and Joint Commission-accredited and is committed to compassionate, high-quality, person-centered care.

Requirements

• Degree from an APTA-accredited Physical Therapy program.
• Active state license to practice as a Physical Therapist.
• At least 1 year of experience or completion of a 6–8 week preceptorship.
• Valid driver’s license and reliable transportation.
• Strong organizational and time-management skills.
• Ability to lift up to 35 pounds and assist with patient mobility.
• Frequent standing, walking, bending, and positioning.
• Ability to adapt to changing schedules and environments.

Nice To Haves

• Home health experience.
• Knowledge of Medicare Conditions of Participation.

Responsibilities

• Conduct comprehensive assessments and develop individualized treatment plans.
• Deliver therapeutic interventions, including manual therapy, modalities, and exercise programs.
• Educate clients, families, and caregivers on therapy techniques and home programs.
• Supervise Physical Therapist Assistants and Home Health Aides as permitted.
• Collaborate with interdisciplinary teams and participate in care conferences.
• Monitor client progress, adjust treatment plans, and communicate changes to physicians.
• Maintain accurate, timely documentation in compliance with regulations.
• Travel to client homes and Trinity Health locations.
