About The Position

Provides therapy to patients based on patient needs and medical direction. Educates patients and caregivers and arranges adaptive equipment as needed. The position evaluates patients, develops and delivers treatment programs, documents care, supports and trains colleagues, contributes to improvement initiatives, and maintains professional skills through continuing education and competency development.

Requirements

• Bachelor's degree or equivalent from an accredited physical therapy program.
• Physical Therapist license in the State of Michigan upon hire.
• Michigan driver's license upon hire for Home Care Business Lines.
• Basic Life Support (BLS) certification from the American Heart Association within 90 days, or Basic Life Support (BLS) certification from the American Red Cross within 90 days.

Nice To Haves

• Master's degree.
• Six months of relevant experience.

Responsibilities

• Evaluate patients to assess disability within the scope of practice, establish goals and a treatment plan, and make appropriate recommendations.
• Provide treatment and education to restore and maximize patient function; assess treatment effectiveness and modify goals or methods as needed.
• Document patient evaluations and treatment according to department standards and medical-legal and reimbursement procedures.
• Train and instruct rehabilitation staff, nursing staff, other medical personnel, and students about patient needs and rehabilitation services.
• Participate in process improvement, patient satisfaction initiatives, program development, and marketing activities.
• Meet the department's productivity standard.
• Develop professional skills through continuing education, in-services, competencies, and development of new patient-care concepts and skills for varied population groups.
