About The Position

The Physical Therapist assists patients in restoring physical function through evaluation and treatment. This home care role involves assessing patients’ physical and functional status, establishing therapy goals, developing treatment plans, observing and documenting patient status, and communicating care plans with patients, families, and other treatment team members.

Requirements

• Doctoral degree in Physical Therapy for those licensed after 2016; those licensed before 2016 are exempt under the stated grandfathering provisions.
• Current Physical Therapy license in the respective state.
• American Heart BLS certification.
• Valid state driver’s license and insurance.
• Demonstrated knowledge and skills necessary to provide patient care appropriate to the population served.
• Strong interpersonal, written, and oral communication skills.
• Analytical ability to evaluate patients, establish goals, formulate treatment plans, observe and document status, and assist technicians and assistants.
• Excellent computer skills and ability to process detailed information accurately.

Nice To Haves

• One year of experience in a hospital or Home Care setting.

Responsibilities

• Evaluate patients’ physical and functional status.

• Communicate with other treatment team members when gathering and exchanging information about patient condition.
• Assist technicians and assistants in resolving problems.
