About The Position

Provides therapy to patients based on patient need and medical direction. Provides patient and caregiver education and arranges for adaptive equipment as needed. This home care role includes evaluating patients, developing and delivering treatment programs, documenting care, educating and training others, and contributing to ongoing improvement and program development.

Requirements

• Bachelor's degree or equivalent from an accredited physical therapy program.
• Physical Therapist license in the State of Michigan upon hire.
• Michigan driver's license for Home Care Business Lines upon hire.
• Basic Life Support (BLS) certification from the American Heart Association within 90 days, or Basic Life Support (BLS) certification from the American Red Cross within 90 days.

Nice To Haves

• Master's degree.
• Six months of relevant experience.

Responsibilities

• Evaluate patients to assess disability within the scope of practice, establish goals and a treatment plan, and make appropriate recommendations.
• Provide treatment and education to restore and return maximal function to patients; assess treatment effectiveness and modify goals or methods as needed.
• Document each patient's evaluation and treatment in compliance with department standards and medical-legal and reimbursement procedures.
• Instruct and train rehabilitation staff, nursing staff, other medical personnel, and students regarding patient needs and rehabilitation services.
• Participate in ongoing process improvement, patient satisfaction initiatives, program development, and marketing activities.
• Meet productivity standards established by the department.
• Improve skills by applying continuing education and in-service learning, participating in applicable competencies, and developing new patient-care concepts and skills for various population groups.
• Provide patient and caregiver education and arrange for adaptive equipment as needed.
