About The Position

The Physical Therapist assists patients in restoring physical function through evaluation and treatment. The role requires evaluating patients’ physical and functional status, setting therapy goals, developing treatment plans, observing and documenting patient status, and communicating care plans with patients, families, and other members of the treatment team. The position is in Home Care.

Requirements

• Doctoral degree in Physical Therapy for those licensed after 2016; those licensed before 2016 are exempt under the stated grandfathering provision.
• State license in Physical Therapy.
• American Heart BLS certification.
• Valid state driver’s license and insurance.
• Demonstrated knowledge and skills necessary to provide patient care appropriate to the population served.
• Strong interpersonal, written, and oral communication skills.
• Analytical ability to evaluate patients, establish goals, formulate treatment plans, observe and document status, and assist technicians and assistants in resolving problems.
• Excellent computer skills and ability to process detailed information accurately.

Nice To Haves

• One year of experience in a hospital or Home Care setting.

Responsibilities

• Evaluate patients’ physical and functional status.
• Establish therapy goals and formulate treatment plans consistent with those goals.
• Observe patients and document their status.
• Assist technicians and assistants in resolving problems.
• Communicate the plan of care and educate family members on care.
• Gather and exchange information about patient condition with other treatment team members.
