Home Care Physical Therapist - $25k bonus
About The Position
Above and Beyond Home Health Care and Hospice Agency, a Trinity Health company, is seeking a Physical Therapist to provide professional, evidence-based care in patients’ homes. The therapist follows physician orders, state regulations, and agency policies, helping clients regain independence and improve quality of life through personalized rehabilitation plans. The position serves the Monticello, Iowa area; candidates may live near Monticello, Cedar Rapids, and/or Dubuque. Travel to client homes and Trinity Health locations is required. Above & Beyond Home Health & Hospice Care is part of Trinity Health At Home, a national, faith-based, not-for-profit organization serving communities across 11 states. The organization is Medicare-certified and Joint Commission-accredited and is committed to compassionate, high-quality care.
Requirements
- Degree from an APTA-accredited Physical Therapy program.
- Active state license to practice as a Physical Therapist.
- At least 1 year of experience or completion of a 6–8 week preceptorship.
- Valid driver’s license and reliable transportation.
- Strong organizational and time-management skills.
- Ability to lift up to 35 lbs and assist with patient mobility.
- Frequent standing, walking, bending, and positioning.
- Ability to adapt to changing schedules and environments.
- Travel to client homes and Trinity Health locations is required.
Nice To Haves
- Home health experience.
- Knowledge of Medicare Conditions of Participation.
Responsibilities
- Conduct comprehensive assessments and develop individualized treatment plans.
- Deliver therapeutic interventions, including manual therapy, modalities, and exercise programs.
- Educate clients, families, and caregivers on therapy techniques and home programs.
- Supervise Physical Therapist Assistants and Home Health Aides, as permitted.
- Collaborate with interdisciplinary teams and participate in care conferences.
- Monitor progress, adjust treatment plans, and communicate changes to physicians.
- Maintain accurate, timely documentation in compliance with regulations.
