About The Position

This is a travel assignment for a Physical Therapist in a short-term acute care hospital setting in Ukiah, California. The contract is for 13 weeks, starting on 09/21/2026. The role involves conducting evaluations, developing and implementing treatment plans, delivering therapy interventions, collaborating with an interdisciplinary team, educating patients and families, and maintaining clinical documentation. Supervision of assistants may also be required.

Requirements

Doctor of Physical Therapy (DPT) degree
Current California Physical Therapy license; active
Basic Life Support (BLS) certification
EPIC EMR experience
Pelvic floor therapy experience with PRPC or WCS certification or actively pursuing certification
Two years of physical therapy experience preferred; 3–5 years preferred for candidates with pelvic floor specialization
Ability to work in a short-term acute care setting; eligible for travel assignment

Responsibilities

Conduct initial and ongoing physical therapy evaluations, documenting assessment findings
Develop and implement individualized treatment plans to restore mobility, strength and function
Deliver therapy interventions using established protocols and therapeutic techniques
Collaborate with an interdisciplinary team; co-lead patient groups as applicable
Educate patients and families; coordinate discharge planning and home exercise programs
Maintain comprehensive clinical documentation within established deadlines; supervise assigned assistants when applicable
