About The Position

This is a travel assignment for an Outpatient Physical Therapist in Fort Bragg, CA, starting on 08/31/2026 and lasting for 13 weeks. Requirements

• Current physical therapy license to practice in California or eligibility for licensure
• Experience in outpatient rehabilitation or a strong interest in outpatient PT practice
• Ability to participate effectively in an interdisciplinary team and collaborative care model
• Availability to start on 08/31/2026 and commit to a 13-week travel assignment
• Willingness to work a 40-hour week on a 4x10 schedule and travel as required

Responsibilities

• Plan and conduct segments of outpatient treatment programs designed to restore and improve physical function, and support social and mental well-being within program objectives.
• Assess patient groups and monitor progress, adjusting therapy plans to meet individualized goals.
• Co-lead interdisciplinary care activities, collaborating with other professionals to coordinate patient care.
• Educate patients and caregivers on treatment strategies, home exercise programs, and safety planning.
• Document patient status, goals, and outcomes in accordance with department policies and regulatory requirements.
• Ensure adherence to approved treatment protocols and maintain patient-centered, evidence-based care.
