About The Position
Relient Health is seeking a compassionate and skilled Physical Therapist Assistant (PTA) for a full-time role at a skilled nursing facility in Hayward, California. The facility provides rehabilitation services through a collaborative, patient-focused therapy department. This is a direct-hire, permanent-placement opportunity with a Monday–Friday schedule. The stated pay range is approximately $45–$50 per hour, depending on experience, setting, and location. Requirements
- Graduate of an accredited Physical Therapist Assistant program.
- Current California license or eligibility to obtain one.
- Strong communication, documentation, and interpersonal skills.

Responsibilities
- Provide skilled therapy services under the direction of a licensed Physical Therapist.
- Assist in developing and implementing individualized treatment plans.
- Document patient progress using electronic medical record (EMR) systems.
- Communicate and collaborate with Physical Therapists, Occupational Therapists, Speech-Language Pathologists, and nursing staff.
- Promote patient independence and mobility.
