About The Position
Relient Health is seeking a compassionate and skilled Physical Therapist Assistant (PTA) to work in a skilled nursing facility in Concord, New Hampshire. This is a full-time, direct-hire/permanent-placement role with a supportive, collaborative, patient-focused therapy team. The facility offers modern equipment and mentorship, and values professional growth. New graduates and experienced PTAs are welcome.
Requirements
- Graduate of an accredited Physical Therapist Assistant program.
- Current New Hampshire license or eligibility to obtain one.
- Strong communication, documentation, and interpersonal skills.
Responsibilities
- Provide skilled therapy services under the direction of a licensed Physical Therapist.
- Assist in developing and implementing individualized treatment plans.
- Document patient progress using EMR systems.
- Communicate with Physical Therapists, Occupational Therapists, Speech-Language Pathologists, and nursing staff.
- Promote patient independence and mobility.
