About The Position

The Patient Transition Coordinator supports seamless care transitions by coordinating referrals to home health services. Working closely with Care Transition Coordinators, Home Health Care Transition Coordinators, Compassus and Bon Secours Home Care intake teams, and external partners, the Coordinator helps ensure patients receive timely and appropriate services. After training, the position is hybrid, with two days at home and three days in the office, or a flexed schedule as required. The usual schedule is Monday through Friday, 8 a.m. to 5 p.m.; flexibility for a weekend rotation is required, which may involve working some weekend hours.

Requirements

• An equivalent combination of education and experience will be considered.
• Ability to perform basic arithmetic, including calculating rates, ratios, and percentages.
• Ability to read, analyze, and interpret business and professional publications, technical procedures, and governmental regulations.
• Ability to write reports, business correspondence, and procedure manuals, and to present information and respond to questions from leaders, team members, investors, and external parties.
• Strong written and verbal communication skills.
• Ability to understand, read, write, and speak English.
• Ability to articulate and embrace hospice philosophy.
• Flexibility to participate in a weekend rotation.

Nice To Haves

• Bachelor’s degree in a health-related field.
• At least one year of experience in a related health care setting.
• One year of experience in home health, hospice, and/or palliative care.
• Prior experience with electronic medical records.
• Knowledge of home health, hospice, and palliative eligibility requirements.
• Previous healthcare experience.
• Knowledge of the EPIC EMR system.

Responsibilities

• Organize, track, and build complete medical records for patient transitions to home health providers, and confirm start of care for each referred patient.
• Identify post-acute care needs and collaborate with the Care Transition Coordinator.
• Collaborate with Compassus and Bon Secours Home Care intake teams.
• Complete transitions of care, including submitting complete referrals and securing placement with an outside agency when Compassus or Bon Secours Home Care cannot provide services.
• Organize and prioritize large volumes of information.
• Provide exceptional customer service to patients, families, and referral sources.
• Handle confidential and non-routine information.
• Create and manage general correspondence, memos, charts, tables, graphs, and business plans.
• Use word processing, spreadsheet, database, and presentation software effectively, including Microsoft Word, Excel, Access, and PowerPoint; compose, type, and proof materials with attention to detail, establish priorities, and meet deadlines.
• Perform other duties as assigned.
