About The Position

PurposeCare is a Midwest provider of home care and home health services, coordinating home-based care for more than 3,000 clients each month. The Care Transition Coordinator is an experienced field marketer responsible for company growth and for developing the organization's Home Health/Home Care presence within supportive living settings and home- and community-based residences. The role generates referrals by building relationships with hospitals, healthcare systems, case managers, and other community resources. The coordinator manages the referral process, including identifying home health needs, verifying benefits, and coordinating care. The role also involves marketing planning, referral-network development, relationship management, and reporting on marketing activities. The professional is expected to represent the agency with care and professionalism, uphold company policies and compliance requirements, maintain confidentiality, and work independently.

Requirements

Be at least 18 years old.
Have one year of experience in home health, home care, marketing, or case management, or a combination of these.
Possess a valid driver's license.
Demonstrate excellent oral, written, and active listening skills.
Be able to work independently, maintain confidentiality, and meet assigned deadlines.
Demonstrate sound judgment and good decision-making skills.
Uphold integrity and fair-mindedness consistent with company standards, practices, policies, and procedures.
Be able to organize and prioritize work.
Be able to establish good working relationships internally and externally.
Be able to sit at a desk and listen effectively for long periods on the telephone.
Present a professional appearance and demeanor.
Be able to use office equipment and have proficient computer skills.
Be patient and friendly.
Be able to pass a criminal background check and drug screen.
Be able to work evenings or weekends as required.

Responsibilities

Participate in short- and long-range planning to achieve company goals and objectives.
Manage the complete referral process, including identifying home health needs, verifying benefits, and coordinating care.
Cultivate new and existing relationships that contribute to company growth.
Represent the agency in a professional, prepared, and caring manner.
Prepare weekly reports on marketing activities.
Initiate and expand the referral network.
Comply with company compliance requirements and HIPAA regulations.
Perform other duties as assigned.
