About The Position

This position provides education and support to screening patients and helps address healthcare disparities and barriers to care. It facilitates communication across the continuum of care, coordinates resources, provides education and advocacy, and coordinates care with managing physicians and the interdisciplinary team. The role exists in various departments throughout the OhioHealth system of hospitals and healthcare services; this opening is in the RMH GME OBGYN department. Duties may also include program coordination, maintaining registries, and collecting and preparing data for metrics reporting.

Requirements

• High school diploma or GED.
• Applicable public health or addiction treatment knowledge and expertise.

• Certification desirable; the type may vary by department.

Responsibilities

• Assess patients’ and families’ learning needs, provide education, assess learning achieved, and provide support and navigation.
• Participate in quality control and assurance.
• Participate in process improvement activities, including collaboration with community organizations.
• Perform department-specific duties, which may include program coordination, maintaining registries, and collecting and preparing data for metrics reporting.
• Perform other duties as requested by the supervisor.
