About The Position

The Patient Care Coordinator is a resource floater who covers different clinics according to need. The role coordinates the functions required to keep patient flow efficient and productive from check-in through treatment and check-out, and serves as the point of contact for non-clinical patient services. The position is with an outpatient physical therapy organization focused on patient-centered care.

Requirements

• High school graduate or equivalent.
• Excellent in-person and phone customer service skills.
• Close attention to detail.
• Strong time management and organizational skills.
• Ability to manage multiple responsibilities at one time.
• Computer knowledge.
• Team-player attitude and an energetic focus on excellent customer service.

Responsibilities

• Greet patients and provide outstanding customer service.
• Coordinate each patient's care from initial evaluation to discharge.
• Manage phone calls at different clinics as needed.
• Perform electronic scheduling.
• Enter data.
• Verify current personal and financial information.
• Maintain patient charts and electronic medical records.
• Collect, post, and deposit patient payments.
• Fax, file, and perform other duties as assigned.
