Patient Care Coordinator (Bilingual English/Spanish)
About The Position
Mindify Wellness and Care is seeking a compassionate, outgoing, bilingual Patient Care Coordinator to help older adults access psychiatric, therapy, and behavioral health services throughout Northern and Central New Jersey. The role focuses on building relationships, educating individuals and families about available services, connecting participants to care, and serving as a trusted community resource. The coordinator works at partner adult day care centers and in community and in-home settings. This is a travel-based position involving coverage across multiple locations, with mileage reimbursement for work-related travel to in-home settings. The role supports patient engagement and continuity of care and includes Spanish-English interpretation when needed.
Requirements
- Fluent in English and Spanish.
- Excellent active listening and interpersonal skills.
- Ability to build relationships and adapt to different environments, policies, and procedures across adult day centers and community settings.
- Comfortable speaking with individuals and groups.
- Valid driver's license and reliable transportation.
- Willingness to travel throughout New Jersey.
- Organized, dependable, and self-motivated.

Responsibilities
- Visit partner adult day care centers throughout New Jersey.
- Conduct in-home visits as needed to support patient engagement and access to care.
- Build relationships with participants, families, and center staff.
- Educate individuals about available behavioral health services.
- Help connect patients with psychiatric and therapy services.
- Support patient engagement and continuity of care.
- Serve as a liaison between patients, families, providers, and center staff.
- Provide Spanish-English interpretation when needed.
- Maintain accurate documentation and uphold HIPAA standards.
