The Patient Care Coordinator is accountable and responsible for managing the clinical care and course of treatment for a number of patients. Care is coordinated in collaboration with physicians, associate staff, and other health care providers to achieve patient outcomes within the projected course of treatment using the nursing process. The role requires age-appropriate care and knowledge of growth and development, including assessing and interpreting patient-status information to identify age-specific needs and provide appropriate care. RN duties may be performed as staffing needs require. The coordinator completes medication reconciliation on admission, discharge, and as needed; documents and calls physicians regarding laboratory and imaging results, changes in status, and resident needs; and reviews charts from iRounds. Unit-specific duties include medication-room checks, resolving medication-dispensing-system discrepancies, checking the code cart, and checking temperature logs for the wipe warmer and medication-room refrigerator, delegating as needed. The role supports efficient unit operations and safe, individualized patient care. Responsibilities include educating patients and families about diagnoses, treatments, prescribed regimens, and medications according to their readiness and ability to learn; participating in physician rounds, Care Team conferences, and QAPI meetings; and completing scheduled pressure-ulcer documentation, daily iRounds documentation in PressGaney, and daily hand-hygiene observations during day-shift hours. The coordinator must be able to navigate the electronic medical record and troubleshoot relevant patient-care equipment and positioning devices.
