About The Position

We are seeking a motivated and energetic individual to join our team as a Patient Care Coordinator and Healthcare Marketer in Utah County. This role is crucial as you will be the primary representative of our Skilled Rehabilitation Facilities, Home health, and Hospice services. Your responsibilities will include developing our business in the Utah county area by meeting with physicians, patients, discharge planners from hospitals and skilled nursing facilities, and other key contacts. Requirements

  • Must be 18 years of age
  • Must have, as a minimum, one (1) year of experience with demonstrated success in a business development or marketing position in a skilled nursing facility, assisted living facility or in another related health care facility.
  • Understanding of customer service principles and exceptional interpersonal skills
  • Healthcare Marketing and sales experience is a must.
  • Must be highly motivated, energetic, and able to communicate effectively with physicians/their representatives, case managers, social workers, admission coordinators and/or other representatives of the community.
  • Working knowledge of Medicare and Medicaid Eligibility rules.
  • Sales and marketing experience, with demonstrated skills and proven results as required by Hospital Organizations.
  • Must be able to receive Reptrax/Vendor Mate credentials
  • Maintain current driver's license, auto liability insurance, and reliable transportation.
  • Ability to read, write, speak and understand the English language.
  • Must be a supportive team member, contribute to and be an example of teamwork.
  • Ability to make independent decisions when circumstances warrant such action.
  • Ability to deal tactfully with personnel, residents, patients, family members, visitors, government agencies/personnel and the general public.
  • Must have patience, tact, and willingness to deal with difficult residents, patients, family and staff.
  • Must be able to relay information concerning a resident or patient’s condition.
  • Must not pose a threat to the health and safety of other individuals in the workplace.
  • Meets general health requirements according to company policy, including medical and physical exams and checking immunity status to various infectious diseases.
  • Ability to assist in evacuation of residents or patients during emergency situations.
  • Agreement to abide by all standards, policies, and procedures of the company, including the company’s compliance and ethics program, is a condition of employment.

Nice To Haves

  • Associates degree in business or marketing is preferred but not required
  • Knowledgeable of insurance coverage and authorization process pertaining to long-term care is helpful.
  • Clinical knowledge is a plus!

Responsibilities

  • Meet with physicians, patients, discharge planners for hospital and skilled nursing facilities, and others in need of our services.
  • Continue to develop our business in the Utah county area.
  • Participate in an open and collaborative relationship between continuum partners (i.e. home health, hospice, SNF, ALF, etc.) working toward shared goals for occupancy census and collections.
  • Perform duties daily as delegated by the Regional Development Director to facilitate the conversion of referrals to admission status.
  • Communicate with external team members regarding referral status and any additional information needed to assist with the admission decision.
  • Communicate daily with central intake, business office, administrator/directors to complete financial evaluations for potential referrals and payment options.
  • Ensure prior authorization is obtained for payment entities that require it.
  • Maintain working knowledge of payors such as other insurances, Medicare A, Medicaid, Private, Veteran’s Administration contracts, and hospital contracts, etc.
  • Assist with processing referrals timely on a daily basis as assigned.
  • Ensure residents or patients are admitted, transferred and discharged from the facility software accurately on a daily basis.
  • Record resident pay status changes on the facility’s internal daily census communication tool.
  • Record resident pay status accurately in the resident’s record at time of admission.
  • Behave in a manner aligned with facility goals for occupancy and customer service.
  • Meet with potential residents/representatives daily and provide information about the facility.
  • Provide tours of the facility daily, and as requested by potential residents/representatives.
  • Work in a helpful and pleasant manner, daily using the telephone to communicate with families, residents, hospitals, insurance companies, etc. related to processing referrals.
  • Work numerous times a day using a computer, electronic medical record, spreadsheets, and other tools related to processing referrals.
  • Complete admission paperwork and files according to facility policy.
  • Inform the Admissions Director of obstacles or barriers to admitting residents.
  • Maintain daily referral tracking information as assigned by the Admissions Director.
  • Communicate with interdisciplinary team members timely regarding room assignments for pending admissions.
  • Demonstrate interpersonal relationship skills by developing and maintaining trust with coworkers.
  • Complete the daily census.
  • Participate in facility assessment activities as needed, such as carrying out duties assigned as part of a performance improvement committee.
  • Collaborate with members of the interdisciplinary team, family members, hospitals and community, etc. in a manner that resolves issues and improves the admissions process.
  • Promote teamwork, mutual respect, and effective communication.
  • Participate in the facility’s plan of correction response to an inspection survey and implement any follow-up as designated by the Admissions Director.
  • Promote safe work practices, safety rules, and accident prevention procedures to prevent employee injury and illness.
  • Treat all residents or patients with dignity and respect.
  • Promote and protect all resident or patient’s rights.
  • Establish a culture of compliance by adhering to all facility policies and procedures.
  • Comply with standards of business conduct, and state/federal regulations and guidelines.
  • Follow appropriate safety and hygiene measures at all times to protect residents, patients and themselves.
  • Maintain confidentiality of protected health information, including verbal, written, and electronic communications.
  • Report noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel.
  • Report any retaliation or discrimination to HR or compliance officer.
  • Report any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents or patients to supervisor and/or administrator.
  • Protect residents or patients from abuse, and cooperate with all investigations.
  • Report any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy.
  • Participate in all life safety and emergency drills and trainings where applicable.
  • Fulfill responsibilities as assigned during implementation or activation of the emergency plan.
  • Report work-related injuries and illnesses immediately to supervisor.
  • Complete all assigned training and skills competency as a condition of employment.
  • Follow established infection control policies and procedures.
