About The Position

Avenues Home Care is hiring Patient Care Assistants to care for elderly, vulnerable, and disabled clients, including people who cannot care for themselves. The PCA is expected to prioritize clients’ needs and provide care according to each client’s intake care plan. The role serves Rossville, Woodland Hills, Lakeview, and other local areas. The position includes assisting clients with activities of daily living, medication assistance, household tasks, meal preparation, shopping, transportation, and companionship. PCAs document and report daily activities and promptly communicate care concerns, changes in condition, suspected abuse or neglect, and unusual events through the agency’s available reporting channels. Caregivers are expected to provide safe, reliable, confidential, and respectful care; follow company policies and procedures; maintain required records; and attend meetings and in-services as requested. The posting lists full-time, part-time, PRN, and short-shift options, with shifts of 8, 10, or 12 hours.

Requirements

Must be at least 18 years old.
A CNA license or PCA license is required.
CPR and First Aid certifications are required.
A negative TB test is required.
Must have a valid driver’s license and automobile insurance.
Must have reliable transportation.
Must maintain a fit-for-duty physical condition and be able to lift or push up to 50 pounds.
Must have good communication skills and be able to follow verbal and written assignments.
Must demonstrate effective interpersonal and communication skills, including tact and diplomacy.
Must demonstrate organizational and planning skills, attention to detail, and follow-through.
Must be able to establish and maintain effective working relationships.
Must maintain confidentiality of work-related information and materials.
Must be able to perform the essential job functions with or without reasonable accommodations.
Must maintain an established work schedule.
Must demonstrate proficiency with the homecare company management system, ClearCare.

Nice To Haves

Prior experience assisting with activities of daily living or caring for elderly, disabled, or other vulnerable clients.
Reliable transportation is listed as preferred, though it is also listed among the minimum requirements.

Responsibilities

Assist clients with activities of daily living according to their plans of care, including bathing, dressing, grooming, hygiene, toileting, transferring, eating, and continence care.
Provide medication assistance, housekeeping, laundry, meal preparation, feeding, shopping, transportation to and from appointments, and companionship as specified in the care plan.
Complete other household tasks specified in the client’s plan of care.
Report daily activity to a supervisor through the agency management system and daily activity logs, using other available communication methods as needed.
Practice safety in all activities involving clients and report unsafe working conditions or behavior.
Provide superior care and remain alert to potential care concerns or changes in a client’s condition.
Report suspected abuse or neglect promptly through the proper chain of command, as required by law.
Document unusual behavior, events, conditions, or people.
Maintain confidentiality of records, correspondence, and other protected information, following company policies and HIPAA-related procedures.
Maintain personal appearance in accordance with company policies, including the dress code.
Arrive on time for shifts and report scheduling changes promptly.
Attend meetings and in-services as requested and abide by company policies and procedures.
Perform other duties as assigned.
