About The Position

The Outreach Specialist supports WellBe’s efforts to acquire new patients and explain the value of its services. This role is the first point of contact for prospective and existing patients, helping them understand the care and benefits available to them and supporting their connection to WellBe’s in-home care model. The specialist works with patients by phone, handling inbound and outbound calls to schedule annual and comprehensive wellness visits, make appointment confirmation calls, and assist with rescheduling or cancellations. The role also answers patient questions and provides the in-home care team with information needed to engage patients in their health. WellBe provides physician-led, advanced-practice clinician-driven geriatric care for frail, poly-chronic older adults, particularly Medicare Advantage patients who may face barriers to accessing care. Care is delivered across the continuum, including in the home, hospital, skilled nursing facility, assisted living, palliative care, and end-of-life care. WellBe’s teams partner with patients’ primary care physicians to provide geriatric medical care and social support in the home and coordinate care across settings. This is a 100% remote role. Candidates must live within 50 miles of Chicago, Atlanta, Columbus, Cincinnati, Rochester, or Pittsburgh. The stated pay range is $20–$24 per hour, based on experience. No supervisory responsibilities or travel are required. The role involves prolonged sitting and the ability to lift up to 20 pounds. Work schedules are assigned during business hours and may be flexible; employees should be willing to adapt to schedule changes and may need to work beyond scheduled hours to support the WellBe model.
