About The Position

DaVita is seeking an organized, self-directed RN to manage Peritoneal Dialysis (PD) patients. The role combines clinical care, patient education, and case management. The nurse educates patients and caregivers in-clinic so they can perform dialysis independently at home, manages a caseload, monitors treatment progress, conducts home visits, and provides on-call support. The position offers Monday–Friday, 8-hour shifts, with weekends and holidays off and flexible scheduling to manage the caseload.

Requirements

• RN license in the state of practice.
• Valid driver’s license, insurance, and reliable transportation.
• Associate Degree in Nursing (ADN).
• Basic computer skills, including MS Word and Outlook.

Nice To Haves

• Bachelor of Science in Nursing (BSN).
• Experience in dialysis, including PD, HHD, chronic, or acute dialysis.
• Patient education or teaching background.
• Case management or telehealth experience.
• CNN or CDN certification.

Responsibilities

• Train patients and caregivers on peritoneal dialysis procedures and self-care.
• Manage a caseload of peritoneal dialysis patients, including assessments and care planning.
• Monitor treatment progress, conduct home visits, and provide on-call support.
• Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes.
