On Call Home Nurse
About The Position
DaVita is seeking an organized, self-directed registered nurse to manage peritoneal dialysis (PD) patients. The role combines clinical care, patient education, and case management. The nurse educates patients and caregivers in the clinic so they can perform dialysis independently at home, manages a caseload, monitors treatment progress, conducts home visits, and provides on-call support. The position is based at 1991 Industrial Dr, Deland, Florida. The schedule is Monday through Friday, with 8-hour shifts and weekends and holidays off. Scheduling is flexible to support management of the nurse’s caseload. The posting date is October 14, 2026. The position will be open for a minimum of three days.
Requirements
- RN license in the state of practice.
- Valid driver’s license, insurance, and reliable transportation.
- Associate Degree in Nursing (ADN) required.
- Basic computer skills, including MS Word and Outlook.
Nice To Haves
- Bachelor of Science in Nursing (BSN) preferred.
- Experience with dialysis, including PD, HHD, chronic, or acute dialysis.
- Patient education or teaching background.
- Case management or telehealth experience.
- CNN or CDN certification.
Responsibilities
- Train patients and caregivers on peritoneal dialysis procedures and self-care.
- Manage a caseload of peritoneal dialysis patients, including assessments and care planning.
- Monitor treatment progress.
- Conduct home visits.
- Provide on-call support.
- Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes.
