On Call Hospice/Home Health Registered Nurse

About The Position
We are seeking a Registered Nurse to join the team. In this role, you will primarily create and manage plans of care for individual patients. The successful candidate is attentive to detail, communicates effectively, and creates personalized care plans in patients’ best interests. The role requires knowledge and skill in current nursing practices and strong problem-solving ability. The schedule is seven days on and seven days off, with work in the evenings and on weekends.
Requirements
- Currently licensed as a Registered Nurse (RN) in the state of employment.
- Excellent interpersonal and client-care skills.
- Valid driver’s license.
- Current first aid/CPR certification is required.
Nice To Haves
- Previous experience as a Registered Nurse is preferred.
Responsibilities
- Coordinate with colleagues to assess, plan, implement, or evaluate patient care plans.
- Administer medications and monitor patients for side effects or reactions.
- Document changes in patient conditions and report those changes.
- Maintain accurate patient-care records and discuss observations with the supervisor.
