On Call Registered Nurse - Hospice
About The Position
The Care Team Hospice is seeking compassionate and experienced On-Call Registered Nurses to provide after-hours and weekend support to hospice patients and families. The role focuses on responding to patient needs, managing symptoms, completing visits when necessary, and supporting patients and families. On-Call RNs do not manage a full daytime caseload. When not actively responding to patient needs in the field, nurses may remain at home while available for on-call responsibilities. The role offers independence and autonomy with support from an experienced clinical team. Flexible scheduling arrangements may include weekday on-call coverage Monday through Thursday from 4:30 PM to 8:00 AM the following morning; weekend coverage from Friday at 4:30 PM through Monday at 8:00 AM; or rotating seven-days-on/seven-days-off coverage. Other arrangements may be discussed based on availability and coverage needs.
Requirements
- Active and unrestricted Registered Nurse (RN) license in the state of practice.
- Minimum of one year of nursing experience.
- Strong clinical assessment, critical-thinking, and communication skills.
- Ability to work independently and confidently respond to changing patient needs.
- Valid driver's license, reliable transportation, and current automobile insurance.
Nice To Haves
- Hospice, home health, oncology, palliative care, or critical care experience preferred.
Responsibilities
- Respond to after-hours patient and family needs, providing nursing assessments, symptom management, and clinical interventions.
- Complete scheduled and urgent patient visits, address changes in condition, and provide compassionate end-of-life care.
- Educate and support patients and caregivers with symptom management, medication needs, and end-of-life transitions.
- Collaborate with physicians, caregivers, and interdisciplinary team members to ensure continuity of care.
- Complete accurate and timely clinical documentation in the electronic medical record (EMR).
