About The Position

James River Hospice is seeking a compassionate, committed Hospice Registered Nurse for on-call work in Lynchburg, Virginia, and surrounding counties. The nurse provides intermittent hospice case management to assigned patients in their residences and coordinates with the interdisciplinary team to support a quality patient-care experience. The schedule is Monday through Friday, 5:00 PM to 8:00 AM.

Requirements

• Current Registered Nurse license to practice in the Commonwealth of Virginia.
• At least one year of hospice experience.
• Graduation from an accredited nursing school.
• Valid driver's license and a reliable car available for use during work hours.

Responsibilities

• Complete an initial assessment of the patient and family to determine hospice needs.
• Develop a care plan that establishes goals based on nursing diagnoses and incorporates palliative nursing actions.
• Coordinate care among the clinical team supporting the patient, and supervise LPNs and hospice aides involved in patient support.
• Spend time with patients to assist with pain management and provide support.
• Modify and re-evaluate the plan of care when necessary.
• Monitor, record, and report symptoms and changes in the patient's condition.
• Obtain and receive physician orders as required.
• Participate in on-call duties as defined by the on-call policy.
• Perform other duties as assigned.
