Requirements

• Licensed Occupational Therapist in the State of Arizona.
• Graduate of an ACOTE-accredited occupational therapy program and eligible for NBCOT certification.
• Minimum of 2 years of clinical experience; home health experience preferred.
• Current CPR certification.
• Valid driver’s license, insurance, and reliable transportation.

• Home health clinical experience.

Responsibilities

• Develop and implement individualized care plans.
• Provide therapy services according to physician orders.
• Assess patients’ functional status and recommend adaptive equipment.
• Teach new skills or retrain patients in activities of daily living.
• Maintain accurate documentation and participate in case conferences.
