About The Position
HealthPro Heritage is hiring a PRN, per diem Occupational Therapist for a long-term care facility in Los Gatos, California. The role involves assessing clients’ physical, emotional, and cognitive abilities; developing individualized treatment plans; educating clients and families; documenting evaluations and progress; and advocating for clients’ needs. The listed compensation range is $70–$73 per hour, depending on experience. Requirements
- Degree in Occupational Therapy from an accredited institution.
- Valid state licensure as an Occupational Therapist, or eligibility for licensure.
- Strong clinical assessment and diagnostic skills.
- Proficiency in developing and implementing effective treatment plans.
- Excellent communication and interpersonal skills with a patient-centered approach.
- Ability to lift and assist patients as needed, stand for extended periods, and perform physical tasks related to therapy.
Responsibilities
- Evaluate clients’ physical, emotional, and cognitive abilities using observations and standardized tools.
- Create personalized treatment plans to meet clients’ goals and enhance their functional abilities.
- Educate clients and their families on improving daily activities, including use of adaptive equipment and environmental modifications.
- Maintain accurate records of evaluations, progress, and treatment plans in compliance with legal and ethical standards.
- Advocate for clients’ needs and support them in accessing community resources, services, and accommodations.
