About The Position
Health Link Home Health is hiring an Occupational Therapist to provide home health services in a dedicated territory. Requirements
- Current California Occupational Therapist license.

Responsibilities
- Complete evaluations, re-evaluations, and discharges, and develop individualized plans of care based on physician orders.
- Provide skilled treatment focused on activities of daily living, adaptive equipment, home safety, and functional independence.
- Educate patients and caregivers on safe, independent routines at home.
- Coordinate with physicians, case managers, and the interdisciplinary team.
- Complete documentation using AI-assisted tools.
- Travel regularly within the assigned territory.
