About The Position
Health Link Home Health & Hospice is hiring an Occupational Therapist to provide home health services in a dedicated territory in San Francisco or San Mateo County. The role may be full-time or part-time, with the territory and schedule designed to minimize drive time. Therapists are paid hourly for scheduled hours, including drive time and documentation, rather than per visit. Compensation is $60–$75 per hour based on experience, with a $10,000 sign-on bonus and productivity bonuses of up to 25% of wages. The employer describes a steady caseload, AI-assisted documentation on company-provided devices, and real-time support from case managers and the clinical team. The Occupational Therapist evaluates patients, develops and delivers individualized plans of care, and supports safe functional independence at home. The role involves patient and caregiver education, coordination with physicians and the interdisciplinary team, documentation, and regular travel within the assigned territory. This is a direct-hire position, not a contract role.
Requirements
- Current California occupational therapy (OT) license.
- Valid California driver's license.
- Ability to travel regularly within the assigned territory.
Nice To Haves
- Home health experience is preferred.
- Strong outpatient, skilled nursing facility (SNF), or hospital experience is welcomed; home health orientation and OASIS training will be provided.
Responsibilities
- Complete evaluations, re-evaluations, and discharges, and develop individualized plans of care based on physician orders.
- Provide skilled treatment focused on activities of daily living (ADLs), adaptive equipment, home safety, and functional independence.
- Educate patients and caregivers on safe, independent routines at home.
- Coordinate with physicians, case managers, and the interdisciplinary team.
- Complete documentation using AI-assisted tools.
- Travel regularly within the assigned territory.
