Occupational Therapist (OT) Home Health - $10K Sign-On Bonus!
About The Position
At Health Link Home Health & Hospice, we believe great patient care starts with supporting the clinicians who deliver it. For over 20 years, we've served the Bay Area with a focus on clinical quality, strong systems, and a collaborative culture. We're growing our team and looking for an OT who wants meaningful work, modern tools, and a team that's truly there for them. You'll be assigned one dedicated territory, either San Francisco (full-time) or San Mateo County (part-time), designed to minimize drive time and keep your day manageable. SF patients are spread across neighborhoods like the Sunset, Richmond, and Mission; Peninsula patients run from Daly City down through Redwood City.
Requirements
- Current California OT license (required)
- Valid California driver's license (required)
- Ability to travel regularly within your assigned territory
Nice To Haves
- Home health experience preferred. Strong outpatient, SNF, or hospital OTs are encouraged to apply, and we'll provide home health orientation and OASIS training
Responsibilities
- Complete evaluations, re-evaluations, and discharges, and develop individualized plans of care based on physician orders
- Provide skilled treatment focused on ADLs, adaptive equipment, home safety, and functional independence
- Educate patients and caregivers on safe, independent routines at home
- Coordinate with physicians, case managers, and the interdisciplinary team
- Complete documentation using AI-assisted tools
