About The Position
Health Link Home Health & Hospice is seeking a Full-Time or Per Diem Home Health Physical Therapist to join their San Francisco team. For over 20 years, Health Link has served the Bay Area with a focus on clinical quality, strong systems, and a collaborative culture. They are looking for a PT who wants meaningful work, modern tools, and a supportive team. The role involves seeing patients across San Francisco's neighborhoods, with a territory designed to minimize drive time and keep the day manageable. Therapists are paid hourly for scheduled hours, including drive time and documentation, with no per-visit pay. AI-assisted charting tools are provided to reduce documentation time.
Requirements
- Current California PT license (required)
- Valid California driver's license (required)
- Ability to travel regularly within San Francisco and surrounding areas
Nice To Haves
- Home health experience preferred
- Strong outpatient, SNF, or hospital PTs are encouraged to apply, and we'll provide home health orientation and OASIS training
Responsibilities
- Complete evaluations, re-evaluations, and discharges, and develop individualized plans of care
- Provide skilled treatment focused on mobility, function, fall prevention, and safety in the home
- Educate patients and caregivers on home exercise programs, equipment, and home safety
- Supervise PTAs on your caseload when assigned
- Coordinate with physicians, case managers, and the interdisciplinary team
- Complete documentation using AI-assisted tools
