Occupational Therapist - Sussex, NJ
About The Position
FOX Rehabilitation is seeking licensed Occupational Therapists to provide one-on-one care to older adults in Sussex, Vernon, Hamburg, and Franklin, New Jersey, and surrounding communities. Care is delivered in private residences and senior living settings, including independent living, assisted living, and memory care, through FOX’s Geriatric House Calls™ model. Clinicians help patients improve mobility, functional independence, wellness, and quality of life. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and provide in-person care with optional telehealth and other virtual services when appropriate. Occupational Therapists work in a clinician-operated, community-based practice and collaborate with physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists. FOX offers full-time, part-time, and PRN/FOX Flex employment pathways. New graduates may be eligible for the Emerging Professionals Mentor Program.
Requirements
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Occupational Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Wear PPE when required.

Responsibilities
- Provide one-on-one occupational therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Create and manage a treatment schedule and build patient relationships.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
