Occupational Therapist - Somerset, NJ
About The Position
FOX Rehabilitation is hiring licensed Occupational Therapists to provide one-on-one care to older adults in Somerset, New Jersey, and surrounding communities. Care is delivered in patients’ homes and senior living settings, including independent living, assisted living, and memory care. Clinicians use FOX’s Geriatric House Calls™ model to help patients improve mobility, functional independence, wellness, and quality of life. Telehealth and other virtual care may also be incorporated when appropriate. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and work in a collaborative, interdisciplinary, community-based practice. FOX offers full-time, part-time, and PRN/FOX Flex employment pathways. Full-time roles are salaried with benefits; part-time roles are salaried with benefits eligibility based on a minimum-hours requirement; PRN/FOX Flex roles are paid per unit. New graduates may be eligible for FOX’s Emerging Professionals Mentor Program. The standard New Jersey base pay range for a full-time role is $70,000–$95,000 annually, with an opportunity to earn more. Part-time and FOX Flex compensation is prorated. Requirements
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Occupational Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Wear personal protective equipment (PPE) when required.

Responsibilities
- Provide one-on-one occupational therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy sessions that support patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
