About The Position

FOX Rehabilitation is hiring licensed Occupational Therapists to provide one-on-one care to older adults in Monroe, New Jersey, and surrounding communities. Clinicians provide care in private residences and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. The role focuses on improving mobility, functional independence, wellness, and quality of life. Clinicians may also incorporate telehealth and other virtual care into their schedules.

The position offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and work collaboratively with an interdisciplinary team. FOX offers full-time, part-time, and PRN/Fox Flex pathways. New graduates may be eligible for the Emerging Professionals Mentor Program. Compensation depends on experience and employment type; the stated New Jersey full-time base pay range is $70,000–$95,000 annually, with the opportunity to earn more. Part-time and FOX Flex compensation is prorated.

Requirements

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
• Ability to drive to patient homes and senior living communities.
• Wear PPE when required.

Responsibilities

• Provide one-on-one occupational therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Provide therapy sessions that support wellness, longevity, mobility, and functional independence.
• Document effectively in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate.
• Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
• Potentially supervise PTA/OTAs, mentor new graduates, serve as a Clinical Instructor, and contribute articles or social content for additional compensation.
