About The Position

FOX Rehabilitation is hiring licensed Occupational Therapists to provide one-on-one care to older adults in Holmdel, New Jersey, and surrounding communities. Care is delivered in private residences and senior living settings, including independent living, assisted living, and memory care. Clinicians help patients improve mobility, independence, wellness, longevity, and quality of life through FOX’s Geriatric House Calls™ model. Telehealth and other virtual care may also be incorporated when appropriate. The role offers autonomy to create, manage, and adjust a treatment schedule and build lasting patient relationships. Occupational Therapists evaluate patients, develop individualized plans of care, deliver evidence-based treatment, document in FOX’s EMR system, and collaborate with an interdisciplinary team. FOX offers full-time, part-time, and PRN/Fox Flex employment pathways. New graduates may be eligible for the Emerging Professionals Mentor Program. FOX Rehabilitation is a clinician-operated practice focused on meaningful relationships, evidence-based care, clinician-driven decision-making, and a mission to abolish ageism. Compensation is based on experience and employment type. New Jersey’s standard base-pay range for a full-time role is $70,000–$95,000 annually, with the opportunity to earn more; part-time and PRN compensation is prorated.

Requirements

Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
Degree from an accredited Occupational Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the stated physical demands of in-home and community-based care, including lifting, CPR, patient transfers and bed mobility, repositioning patients, extended standing and walking, climbing stairs, and performing therapeutic activities.
Ability to drive to patient homes and senior living communities.
Ability to wear PPE when required.

Responsibilities

Provide one-on-one occupational therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Conduct therapy sessions that support patient wellness, longevity, mobility, and functional independence.
Document care in FOX’s EMR system in accordance with regulatory standards.
Collaborate with physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
Potentially supervise PTA/OTAs, mentor new graduates, teach as a Clinical Instructor, and contribute written or social content for additional compensation.
