About The Position
FOX Rehabilitation is hiring licensed Occupational Therapists in Carmel, Indiana, to provide one-on-one care to older adults in private residences and senior living communities. Using the Geriatric House Calls™ model, clinicians help patients improve mobility, independence, and quality of life. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Clinicians may also provide telehealth and other virtual care when appropriate. The position includes evaluating patients, developing individualized plans of care, delivering evidence-based treatment, documenting in FOX’s EMR system, and collaborating with an interdisciplinary team. Care settings include private residences, independent living, assisted living, memory care, and other senior living communities. FOX lists full-time, part-time, and PRN / FOX Flex employment pathways. Compensation varies by experience and employment type. Benefits are available for eligible roles. New graduates may be eligible for FOX’s Emerging Professionals Mentor Program.
Requirements
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Occupational Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, CPR, patient transfers and bed mobility, repositioning patients, extended standing and walking, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Ability to wear PPE when required.

Responsibilities
- Provide one-on-one occupational therapy to older adults in private residences and senior living communities.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
- Document in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
- Potentially supervise PTA/OTAs, mentor emerging professionals, serve as a Clinical Instructor, or contribute written or social content for additional compensation.
