About The Position
Powerback is hiring a full-time Occupational Therapist for Santa Clara Post Acute, a 201-bed skilled nursing facility with a high-volume, diverse post-acute caseload. Patients may need care related to orthopedic joint replacements, pulmonary conditions, or stroke and neurological recovery. The facility has a well-equipped gym and open courtyard spaces for functional therapies. The Occupational Therapist assesses patient needs, develops recovery plans, and delivers occupational therapy designed to restore, reinforce, and enhance patients’ physical abilities and independence. The role involves individualized, patient-centered care, adapting to varying patient needs and care settings, collaborating with peers, making referrals when appropriate, and teaching patients and caregivers skills that promote independence and productivity. Requirements
- Bachelor’s degree in an occupational therapy curriculum accredited by ACOTE standards.
- Initial registration from the National Board for Certification in Occupational Therapy.
- Valid and unencumbered occupational therapy license as required in the state of practice.
Responsibilities
- Assess patient needs.
- Develop patient recovery plans.
- Deliver occupational therapy to help patients regain and improve physical abilities and independence.
- Adapt therapy to individual patient needs and varied care settings.
- Collaborate with peers and make referrals to help patients access appropriate care.
