About The Position

Adaptive Home Health is building a higher-acuity, patient-centered skilled home health model across Texas, with a mission to improve patient access to home health care. The Occupational Therapist is a front-line clinician who restores function in patients’ homes, develops safe routines, reduces avoidable decline, and helps patients regain independence in activities of daily living (ADLs) and instrumental activities of daily living (IADLs). The role involves in-home evaluations and skilled OT visits, developing and carrying out individualized plans of care, recommending and teaching the use of adaptive equipment and home modifications, and coordinating with nursing and the broader care team when patient needs or condition change. The clinician documents care in Homecare Homebase (HCHB). The position is field-based and offers autonomy, a productivity-point compensation model, and operational and technology support intended to reduce administrative friction.
