Home Health Occupational Therapist (OT) Opportunity in: Green Valley and Sahuarita, Arizona. Status: Part-Time (Part time Minimum Expectation of 5 visits or 10 hours per week). Rates: $70 - $95 per visit depending on experience, PLUS mileage and travel time. Embark on a new career path where therapy is customized to a patient’s life and outcomes are optimized. Requirements

• Current licensure or certification as an Occupational Therapist (OT) in the state of Arizona.
• Graduate of an accredited Occupational Therapist program
• CPR Certification (demonstration course required)
• Valid Arizona Fingerprint Clearance Card

Nice To Haves

• Experience in home health preferred

Responsibilities

• Provide occupational therapy to patients unable to perform daily tasks due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT and SLP, focused on the patient’s plan of care and personal goals
• Assess patient, observe deficits, establish therapy goals and document progress
• Assist patient with exercises to improve fine motor skills and coordination
• Suggest adaptive equipment such as grab bars and shower chairs to offer added support
• Assess fall risks and introduce strategies to improve home safety
• Educate patient and family on plan of care, exercises, goals and self-care
