About The Position
FOX Rehabilitation is seeking licensed Occupational Therapists to provide one-on-one care to older adults in private residences and senior living communities in Edison, New Jersey, and surrounding communities. Using FOX’s Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. Care may also include telehealth and other virtual services when appropriate. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and work in a community-based, interdisciplinary practice. FOX offers full-time, part-time, and PRN/FOX Flex pathways; new graduates may be eligible for the Emerging Professionals Mentor Program. Full-time roles include benefits. Part-time benefits eligibility depends on minimum hours requirements, and FOX Flex is compensated per unit. The standard New Jersey base pay range for a full-time role is $70,000–$95,000 annually, with an opportunity to earn more. Part-time and FOX Flex compensation is prorated. Requirements
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Occupational Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Wear PPE when required.

Responsibilities
- Provide one-on-one occupational therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
- Document effectively in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
